your personal property in which Fundamentals 4 LLC conducts your training. 

PARTICIPANT/ATTENDEE

In consideration of the opportunity to participate with Fundamentals 4 LLC Trainings, held at

either of the above locations.

I, __________________Digital Signature________________, as an attendee/participant do hereby agree to the terms listed below.

I understand and acknowledge the inherent dangerous nature of Firearms Trainings, including the use of live ammunition, and I hereby assume all risk of personal injury or death, and property damage or loss, from whatever causes arise from my participation in or attendance of Fundamentals 4 LLC training at either of the above locations

I, for my heirs, executors and administrators, release and forever discharge Fundamentals 4 LLC training at, at either of the above locations, their agents, employees, volunteers, representatives, successors and assigns of all liabilities, claims, actions, damages, costs or expenses which I may have against them arising out of or in any way connected with my participation in or attendance at this event. I understand this waiver includes any claims based on negligence, action, or inaction of any of the parties, including Fundamentals 4 LLC at either of the above locations.

I hereby release, waive, discharge and covenant not to sue Fundamentals 4 LLC, at either of the above locations.

from all liability to the undersigned, their personal representatives, assigns, heirs and the next of kin for any and all loss or damage and any claim or demands therefore on account of injury to the person or property or resulting in death of the undersigned arising out of participating in or attending this event, whether caused by the negligence of Fundamentals 4 LLC at either of the above locations.

I hereby assume full responsibility for and risk of bodily injury, death, or property damage due to the negligence of Fundamentals 4 LLC, at  the above locations.or otherwise while engaged in this event.

I further understand and agree my participation in or attendance at Fundamentals 4 LLC training at either of the above locations.
